HIPAA Notice of Privacy Practices

This Notice of Privacy Practices describes how Steve Antoine, LCSWS (“the Practice”) may use and disclose your protected health information (PHI) and how you can access this information. Please review it carefully.

Our Responsibilities

The Practice is required by law to maintain the privacy of your PHI, provide you with this notice of our legal duties and privacy practices, and notify you in the event of a breach of your unsecured PHI.

How We May Use Your Information

We may use and disclose your PHI for treatment, payment, and healthcare operations purposes. We may also use or disclose your PHI when required by law, in emergency circumstances, or with your written authorization for other purposes.

Your Rights

You have the right to request access to your PHI, request corrections, request restrictions on certain uses and disclosures, request confidential communications, receive an accounting of disclosures, and receive a paper copy of this notice.

Website and Digital Communications

The contact form on this website is not a secure clinical communication channel and should not be used to transmit PHI. Do not include diagnosis, symptoms, medications, or treatment details in website form submissions. Secure communication channels will be established at the outset of the therapeutic relationship.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with the Practice or with the U.S. Department of Health and Human Services Office for Civil Rights. You will not be retaliated against for filing a complaint.

This Notice applies to clinical services provided by Steve Antoine, LCSWS. It does not apply to the purchase of therapeutic workbooks, which is governed by the License Terms. We reserve the right to change this Notice and will make the revised Notice available upon request.
